ATTENDING SCHOOL VERIFICATION

This Form is to be signed by the Principal or some Administrative Staff at the School these players attend. This is to verify that all the players listed on this sheet go to the school they represent and the team they are playing for! This is needed so we can verify on or before the day of the tournament that this team is not an ALL-STAR or AAU Team, our Tournament is being listed and ran as a school team tournament. We want this to be fair to all parties involved and are sorry for this inconvenience, but have had teams in the past whom have snuck in ALL-STAR and AAU teams just to win the Tournament. I don’t see how this could be Fun or enjoyable for those types of Teams to beat up on school teams, for just an Award. This is why I have incorporated this form into my Registration. For Future References and those teams will never be permitted to play again!

Players must be Eligible to Play High School Sports, per OHSAA for the Grade they represent; Example a 3^rd grader can’t be 10 before Aug of 2011 4^th grader can’t be 11 before Aug of 2011, 5^th grader can’t be 12 before Aug of 2011, 6^th grader can’t be 13 before Aug of 2011. They age that makes them eligible to play High School Sports their Senior Year, (Check OHSAA guidelines).
